Registration Guidelines

Applications are accepted at the school on an on-going basis.  Please check the Oasis Academy website (link below) to fill out all of the appropriate forms.  Class openings are filled on a lottery basis.

                                    Student Information Form

                                    Student Health Needs Form

                                    Student Special Needs Form

                                    Family Commitment Form

                                    Authorization to Release Confidential Info. Form
